Enrollment Procedure

To secure a position for your child:

1.  Schedule a meeting with the Director and tour Open Arms CDC.

2. Complete the following forms: Registration, Child Schedule, Child Enrollment, Health History, Day Care Immunization Record, Child Health Record, DPI-Child Care Food Program paper work, and Intake Information form for children less than two years.

3. Payment of the non-refundable registration fee of $50.00 per child.

4. Read and sign the form at the end of the Open Arms CDC Parent Handbook stating that you have read, understand, and will comply with the policies listed in the handbook.

5.  Review sign-in and sign-out procedures with the Director.

6.  Enrollment procedures must be fully completed before a child may attend Open Arms CDC.
